issue-tracker

A shared system for managing software issues using GitHub Issues for official work and local Markdown files for drafts. GitHub Issues are online records of bugs, tasks, and decisions in a code project.

In plain words
What is it for?
It helps draft, promote, read, list, comment on, and label issues, as well as use pull requests for triage. It also defines conventions for keeping local and GitHub records connected.
Why use it?
It separates ideas that are still being shaped from issues ready for the team to track. This keeps the official tracker focused while preserving early notes and ticket breakdowns.

Getting it into your agent

One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

git clone --depth 1 https://github.com/govza/mlo-mcp
Per session 0 Only the description is in the session, so the agent can decide to use it. The body loads when it is invoked.
When invoked 964 The whole file, excluding the scripts and references it only reads on demand.
Security scan A 0 findings. Scan, not verified.
Origin original No closer match found in the catalogue.
Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5 $0.00000 $0.00964
Opus 5 $0.00000 $0.00482
Sonnet 5 $0.00000 $0.00193
Haiku 4.5 $0.00000 $0.00096

Measured yesterday against content hash 1c0c48def8a7, method: parsed. Prices are Anthropic first-party input rates as of 2026-08-30, from the pricing page.

Security

Grade A, and why

issue-tracker scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured yesterday.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 56 lines — stays where its author put it; the contents beside it link to each section on GitHub.

Issue tracker: GitHub + local markdown (hybrid)

This repo uses two tiers:

  • GitHub Issues (govza/mlo-mcp, via the gh CLI) — the official tracker. Settled, actionable issues live here.
  • Local markdown under .scratch/ (gitignored) — work-in-progress: draft specs, exploratory ticket breakdowns, and wayfinding efforts that aren't ready to be official.

Rule of thumb: work starts in .scratch/ while it's still being shaped; once an issue is fully specified and worth tracking officially, promote it to a GitHub issue (and note the GitHub number back in the local file, or delete the local file).

GitHub conventions

  • Create an issue: gh issue create --title "..." --body "...". Use a heredoc for multi-line bodies.
  • Read an issue: gh issue view <number> --comments, filtering comments by jq and also fetching labels.
  • List issues: gh issue list --state open --json number, title, body, labels, comments --jq '[.[] | {number, title, body, labels: [.labels[].name], comments: [.comments[].body]}]' with appropriate --label and --state filters.
  • Comment on an issue: gh issue comment <number> --body "..."
  • Apply / remove labels: gh issue edit <number> --add-label "..." / --remove-label "..."
  • Close: gh issue close <number> --comment "..."

Infer the repo from git remote -vgh does this automatically when run inside a clone.

Local markdown conventions

  • One feature per directory: .scratch/<feature-slug>/
  • The spec is .scratch/<feature-slug>/spec.md
  • Implementation issues are one file per ticket at .scratch/<feature-slug>/issues/<NN>-<slug>.md, numbered from 01 — never a single combined tickets file
  • Triage state is recorded as a Status: line near the top of each issue file, using the role strings from triage-labels.md
  • Comments and conversation history append to the bottom of the file under a ## Comments heading

.scratch/ is gitignored — never commit its contents.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

issue-tracker is an agent published in the GitHub repository govza/mlo-mcp (1 stars, last pushed 13d ago), licensed MIT. It costs nothing until one of its globs matches a file; then it loads 964 tokens. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
