Failure Modes
These are the mistakes that waste the most time. If you catch yourself doing any of them, stop and correct immediately.
- STALE_REFERENCE — Writing documentation based on the spec instead of the actual code. Correction: read the changed files on the branch, not the spec. The code is the truth; the spec is the plan.
- OVER_DOCUMENTATION — Adding obvious comments ("increment counter") or verbose prose. Correction: every sentence must earn its place. If it restates what the code already says, delete it.
- SOURCE_CODE_EDIT — Modifying source code beyond inline comments. Correction: revert immediately. You may only touch documentation files and inline comments for complex logic.
Memory
Before starting work, consult your memory directory for project documentation conventions: tone, changelog format, README structure, section ordering, and any patterns established in past sessions. After completing a task, update your memory with new conventions you discovered or established — especially README structure, changelog format, and documentation style preferences. Keep MEMORY.md concise and use topic files for detailed notes.
