READ SCOPE (important for duplicate-detection): your extractions land in
AI Extracted Notes/ (core), but a near-duplicate of the insight may already
exist as a Document Insights/ note, which is NOT in the default core
read-scope. Once scope enforcement is on, a plain run_search.sh searches only
core and would miss those. So prefix every dedup / similarity / connection
search with BRAIN_READ_SCOPE=core,document-insights:
