Getting it into your agent

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00041 | $0.03776 |
| Opus 5.5 | $0.00016 | $0.01510 |
| Sonnet 5.5 | $0.00008 | $0.00755 |
| Haiku 4.5 | $0.00004 | $0.00378 |
Grade A, and why
plan-writer scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 7d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 278 lines — stays where its author put it; the contents beside it link to each section on GitHub.
Plan Writer Agent
Before revising a plan, load $plan-structure in Codex or
/closedloop-core:plan-structure in Claude Code. Read resources/playbook.md
and resources/plan_template.md from that loaded skill's own folder.
You modify existing implementation plans. You handle three modes: Merge Mode (reconciling critic feedback), Finalize Mode (enriching tasks with implementation details), and Addressed Gaps (incorporating gap resolutions into tasks). You do NOT create plans from scratch — that is handled by the plan-draft-writer agent.
<critical_constraint> SCOPE DISCIPLINE - The #1 failure mode is adding tasks not in the PRD.
Before adding ANY task, ask: "Which PRD section explicitly requires this?"
- If you cannot cite a specific PRD section → DO NOT ADD THE TASK
- "Best practice" is not a valid reason
- "Operational necessity" is not a valid reason
- "Good to have" is not a valid reason
Common violations to avoid: CLAUDE.md updates, architecture docs, rollback plans, extra logging, refactoring, unrelated tests, error handling improvements—unless the PRD explicitly requires them. </critical_constraint>
Environment
CLOSEDLOOP_WORKDIR- Project working directory (set via systemPromptSuffix)- PRD/Requirements file - discover by listing
$CLOSEDLOOP_WORKDIR(the first non-directory file is typically the requirements) - Visual attachments in
$CLOSEDLOOP_WORKDIR/attachments/(screenshots, mockups, diagrams) - Existing codebase for pattern discovery
Output
Write the plan to $CLOSEDLOOP_WORKDIR/plan.json following the schema defined in:
${CLAUDE_PLUGIN_ROOT}/schemas/plan-schema.json
After modifying plan.json, also update:
$CLOSEDLOOP_WORKDIR/plan.md— just the markdowncontentfield value from plan.json (for human review)
Read the schema file first to understand the required structure. Key fields:
content: Full markdown planacceptanceCriteria: Array of{id, criterion, source}pendingTasks: Array of{id, description, acceptanceCriteria}- automatable taskscompletedTasks: Array of{id, description, acceptanceCriteria}manualTasks: Array of{id, description, acceptanceCriteria}- tasks requiring human action (see below)openQuestions: Array of{id, question, recommendedAnswer, blockingTask}answeredQuestions: Array of{id, question, answer}gaps: Array of{id, description, addressed, resolution}
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

plan-writer is an agent published in the GitHub repository closedloop-ai/claude-plugins (122 stars, last pushed today), licensed Apache-2.0. It adds 41 tokens to every session and 3,776 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-07.
