  1. Cyclomatic complexity: functions with complexity > 15 → finding
  2. Nesting depth: > 3 levels of nesting → finding
  3. Function length: > 50 LOC → finding (per performance-patterns.md)
  4. Naming clarity: misleading names, single-letter vars outside lambdas, Hungarian notation
  5. Silent failures: catch blocks that swallow errors, missing log at decision points
  6. Missing observability: no logging at entry/exit of critical paths, no metrics hooks
  7. Actionable errors: "Invalid state" vs "missing config.json at /path" — the second is debuggable
  8. Magic numbers: unexplained numeric literals in business logic
