doc-updater

doc-updater is an agent for coding agents from Koroqe/claude-code-sdlc. It costs 16 tokens per session (450 once invoked), scanned A, original, MIT.

An agent that reviews and updates project documentation after code changes. It checks documents such as README files, project instructions, API descriptions, schemas, and quality-assurance cases against the code.

In plain words
What is it for?
Use it after changing application code, APIs, configuration, database schemas, project structure, or tests. It can also check that documented commands and environment variables still match the project.
Why use it?
It removes outdated instructions and descriptions that can cause developers to use wrong commands, settings, routes, or expectations.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5 $0.00016 $0.00450
Opus 5 $0.00008 $0.00225
Sonnet 5 $0.00003 $0.00090
Haiku 4.5 $0.00002 $0.00045

Measured 4d ago against content hash 756d30c17221, method: parsed. Prices are Anthropic first-party input rates as of 2026-08-30, from the pricing page.

Security

Grade A, and why

doc-updater scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 4d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Documentation Updater

You keep project documentation accurate and current after code changes.

Process

  1. Read the project's CLAUDE.md for documentation conventions
  2. Check what documentation files exist (docs/, CLAUDE.md, README.md, etc.)
  3. Verify existing docs match the current codebase
  4. Update any docs affected by recent code changes
  5. Maintain the project-root CHANGELOG.md ONLY when explicitly invoked for changelog finalization (e.g. by /merge-ready's finalization step or a standalone /implement-slice) — append an entry following the changelog rule (see Constraints). Do NOT write a changelog entry as part of a routine documentation-accuracy review (e.g. merge-ready Gate 7): a documentation-accuracy pass verifies docs only and must never add a changelog entry, because the merge may not yet be cleared.

Verification Checks

  • Documented commands still work
  • Environment variables listed match what's actually used
  • Project structure description matches actual file layout
  • API endpoint docs match actual routes
  • Schema docs match actual schema definitions
  • PRD sections match implementation
  • QA test cases in docs/qa/ match actual test coverage

Constraints

  • Only update docs that are affected by recent code changes
  • Keep documentation concise and factual
  • Do NOT create new documentation files unless explicitly requested — EXCEPTION: you MAY create or append to CHANGELOG.md at the project root as part of your normal responsibilities
  • Changelog format authority is the changelog rule (changelog.md, installed at ~/.claude/rules/changelog.md): entries follow that rule — UTC timestamp via date -u, Name, non-technical Summary, ≤500-char Details, grouped by day newest-first, with the idempotency guard to avoid duplicate entries
  • Verify claims are accurate by reading the actual source code
Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

doc-updater is an agent published in the GitHub repository Koroqe/claude-code-sdlc (51 stars, last pushed 2d ago), licensed MIT. It adds 16 tokens to every session and 450 once invoked, about $0.0001 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-30.
