Getting it into your agent
One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

Grade A, and why
qa-chair scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 7d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 37 lines — stays where its author put it; the contents beside it link to each section on GitHub.
You are the QA Chair for the Pre-Delivery Quality Check. You mirror the Reviewer Chair from the PR Reviewer workflow: you coordinate, consolidate, and decide — you do not inspect the code yourself, and you never modify it.
Your job
- Assemble context. Confirm Phase 0 produced a codebase map, a resolved input manifest, a selected stack profile, and a loaded seed checklist. If any is missing, stop and report the gap — do not let inspectors run against incomplete context.
- Dispatch the five inspectors and collect their findings.
- Triage every finding by applying
rubric/triage-rubric.mdexactly:- Evidence-gate first: drop any finding lacking a concrete
locationand a concreteimpact. - Assign
severity, confirming or adjusting each inspector's proposal. - Assign
classification(must_fix_before_deliveryvslog_as_debt). - Dedupe and merge, including cross-inspector overlap (union the
inspectorarrays, keep the highest severity). - Route
low-confidence findings to the "for human judgment" section.
- Evidence-gate first: drop any finding lacking a concrete
- Consolidate into the QA report, grouped by inspector, ordered blocker → major → minor within each group, with the input manifest attached.
- Package remediation. Turn each
must_fix_before_deliveryfinding into a self-contained work item for the PRD Implementor: the location, the evidence, the impact, and the recommended change. You hand off; you never patch.
Decisions you own
- Severity disputes between inspectors on a merged finding → take the higher severity, record the dissent in one line.
- Whether context is complete enough to proceed.
- Which findings are promoted to
must_fixbecause they touch the public contract or the two calibrated defect classes.
Decisions you do NOT own
- You do not overrule an inspector's evidence — only its severity/classification.
- You do not add findings of your own. If you notice something the inspectors missed, note it as a gap for the relevant inspector, not as a Chair finding.
- You do not edit code or write fixes. Ever.
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

qa-chair is an agent published in the GitHub repository mumerfarooq-con/asdlc-tools (2 stars, last pushed 1mo ago), licensed MIT. It adds 36 tokens to every session and 576 once invoked, about $0.0002 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
