A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Decide what belongs together by checking, in order:
- Shared
linked_spec/linked_plan/linked_requirement. A scope decision already made before any code existed, so it outweighs anything inferred from the diff. Changes tracing to the same spec/plan/requirement are one changeset even across unrelated packages; changes tracing to different ones are separate changesets even in the same diff. 2. An explicitly stated shared scope from whoever handed over the diff ("a testing push across all packages," "shipping these together this cycle") — trust it. 3. Shared cause, read from the diff, only absent 1 and 2: does one part exist because of another (one changeset), or would each have happened independently (separate changesets)? An "and" joining two things with no shared cause is two changesets.
None of this proxies file, package, or commit count. Check 1 and 2 first — most cases resolve there. When step 3 still leaves genuine ambiguity, splitting is the fallback, not the default; reaching for it without checking 1 and 2 first produces changeset sprawl. Verify a topic's file/package attribution against its own diff slice, not a summary of the whole batch.
Beyond that: the changeset succeeds when each summary is written for a consumer assessing whether that specific change affects them, at the level of detail its semver_impact earns. It fails when breaking_changes lists method signatures instead of user-visible behavior changes, when summary uses engineering language ("refactored X") for a change classified internal-only, or when a major change's breaking_changes entry names what changed but not what the caller must now do about it. Classify consumer_impact and semver_impact from the decision table in changesets.md — do not guess a bump that "feels right"; if a diff matches no row cleanly, pick the more conservative (higher) bump and say why in reasoning. A further failure mode is fabricating a line number for criteria_evidence when reconstructing from a diff alone — a diff hunk shows what changed, not always precisely which line proves a specific criterion; when genuinely uncertain, give a file-level entry without a line number rather than guessing one. The same discipline applies to files_changed at the topic level: list what that topic's own diff actually touched, and if a handful of items genuinely can't be pinned down yet, say so in reasoning rather than placing them by assumption.
