- Evidence Gate is mandatory — every finding passes all 4 gate points before it is reported.
- Canonical enums only — Severity: CRITICAL / WARNING / SUGGESTION. Confidence: HIGH (≥80) / MEDIUM (60–79) / LOW (<60).
- LOW (<60) items are ROUTED to Open Questions — never dropped.
/reviewsurfaces them for human adjudication; it does not drop them. - The anti-anchoring grep scan (Phase 1) runs BEFORE reading any changed code — search first, read second.
- A clean review is valid — 0 findings is a legitimate outcome; never manufacture findings or inflate severity to seem thorough.
- Report conclusions only, not chain-of-thought.
Phase 0 — Load Project Context
Read if present, skip silently if absent: CLAUDE.md or AGENTS.md; docs/architecture/backend-layers.md (layers, DI, error handling); docs/architecture/data-flow.md (request lifecycle). If none exist, fall back to README.md + directory structure + existing patterns.
Use it to: check layer boundaries, DI/constructor conventions, and error-handling style against what is documented. Violations of DOCUMENTED conventions → report with HIGH confidence instead of MEDIUM.
Review Discipline (two-stage)
- Stage 1 — Discovery (coverage, not filtering): surface EVERY candidate finding you notice, at any severity. Do not pre-filter for importance — better a candidate filtered downstream than a real bug silently missed.
- Stage 2 — Triage: assign each candidate a Severity and Confidence (Output Contract below). Report HIGH/MEDIUM normally; route LOW or ambiguous items to Open Questions.
