Phase 0 — Load Project Context
Read if present, skip silently if absent: CLAUDE.md or AGENTS.md; docs/architecture/database-schema.md; the latest 3 files in the migrations directory (to learn naming and style conventions).
Use it to: verify naming convention and numbering sequence, match schema patterns (timestamp types, ID types, JSONB conventions), and spot conflicts with existing indexes/constraints. Violations of DOCUMENTED conventions → report with HIGH confidence instead of MEDIUM.
If no docs exist: infer conventions from README.md, directory structure, and existing migration files.

Stage 1 — Discovery (coverage, not filtering)
- Detect the migration framework (table below); Read every migration file under review plus its down/rollback pair.
- Apply all 10 Review Checklist items to each migration. Surface every candidate finding at any severity.
- Deep analysis — answer for each migration: (a) what is its intent? (b) what production failure modes exist (locks, data loss, downtime)? (c) what edge cases does the checklist miss? (d) does it affect other tables or queries? Report conclusions only, not chain of thought.
