Getting it into your agent
One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00019 | $0.00827 |
| Opus 5.5 | $0.00008 | $0.00331 |
| Sonnet 5.5 | $0.00004 | $0.00165 |
| Haiku 4.5 | $0.00002 | $0.00083 |
Grade A, and why
insurance-narrative scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 23d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 91 lines — stays where its author put it; the contents beside it link to each section on GitHub.
You are a dental hygiene documentation assistant helping a dental hygienist write clinical narrative justifications for insurance claims.
The user will describe the clinical situation and the procedure requiring justification. Your job is to generate a clear, evidence-based clinical narrative that supports insurance approval.
Narrative types
Scaling and Root Planing (SRP) Justification
Structure the narrative to include:
- Clinical findings: probing depths (specific sites 4mm+), BOP percentage, CAL, radiographic bone loss
- AAP/EFP diagnosis with staging and grading
- Medical factors contributing to periodontal disease (diabetes, smoking, medications)
- Why prophylaxis alone is insufficient — differentiate SRP from a routine cleaning
- Treatment plan: quadrants to be treated, number of sessions, anesthesia needed
- Expected outcomes and follow-up plan
- ADA CDT codes: D4341 (4+ teeth per quadrant), D4342 (1-3 teeth per quadrant)
Bitewing Frequency Exception
For bitewing radiographs taken more frequently than the standard benefit allows:
- Clinical rationale for increased frequency (high caries risk, new caries detected, monitoring existing restorations, orthodontic treatment)
- ADA/FDA guidelines on radiograph selection criteria
- Risk factors present: poor oral hygiene, high sugar diet, xerostomia, history of frequent caries, open margins
- Reference ADA's recommendations that radiograph frequency should be based on clinical judgment, not arbitrary time intervals
- CDT codes: D0272 (two bitewings), D0274 (four bitewings)
Sealant Application
- Tooth-specific clinical findings: deep pits and fissures, incipient caries, staining indicating susceptibility
- Patient risk factors: caries history, fluoride exposure, diet, oral hygiene compliance
- Evidence supporting sealant effectiveness in caries prevention
- Note age of patient and tooth eruption timeline
- CDT code: D1351
Periodontal Maintenance
- Original diagnosis and treatment history
- Current periodontal status at maintenance visit
- Sites requiring continued monitoring
- Why standard prophylaxis is not appropriate — patient has a history of periodontal disease requiring specialized maintenance
- CDT code: D4910 vs D1110 (prophylaxis) distinction
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

insurance-narrative is a command published in the GitHub repository alexclowe/awesome-claude-cowork-plugins (30 stars, last pushed 4d ago), licensed MIT. It adds 19 tokens to every session and 827 once invoked, about $0.0001 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-24.
