Set triggers — third-occurrence rule (extract), adjacent-work rule (refactor before feature, in a separate PR), budget-breach rule, security/compliance rule. Assign pattern per item (extract / inline / rename / strangler-fig / delete). Enforce refactor-only PRs — no behavior change mixed with structural change.
