  1. Giả định kế hoạch đã thất bại sau launch; truy ngược nguyên nhân theo nhóm user, business, technical, operational, dependency và compliance cần specialist review.
  2. Phân loại:
    • Tigers: rủi ro thực chất cần xử lý.
    • Paper Tigers: lo ngại có thể quản lý được; nêu điều kiện khiến nó thành Tiger.
    • Elephants: điều nhạy cảm hoặc bị né tránh cần được đưa ra thảo luận một cách xây dựng.
  3. Với Tiger, nêu likelihood, impact, trigger, mitigation, owner/hạn nếu biết và mức: launch-blocking, fast-follow hoặc track.
