What is it for?
Assess workplace risks, record problems such as neck pain or eye strain, and review the ergonomics of chairs, monitors, keyboards, and desks.
Why use it?
It gives workers a structured way to record work-related symptoms and review possible risks in their job or workstation. Professional medical advice is still needed for diagnosis and treatment.
