fresh-eyes-review: Instructions file for Claude Code

CLAUDE.md

fresh-eyes-review CLAUDE.md is an instructions file for Claude Code from 2389-research/fresh-eyes-review. It costs 814 tokens per session, scanned A, original, MIT.

A required final code review that checks changes for security problems, logic mistakes, input-handling gaps, performance issues, and business-rule errors before they are shipped.

In plain words
What is it for?
Reviewing code before commits, pull requests, deployment, or declaring a task complete; it also fixes found issues and reruns tests.
Why use it?
It provides a second review after tests pass, helping catch defects that automated tests and the original author may miss.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00814 $0.00814
Opus 5 $0.00407 $0.00407
Sonnet 5 $0.00163 $0.00163
Haiku 4.5 $0.00081 $0.00081

Security

Grade A, and why

fresh-eyes-review CLAUDE.md scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 11d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Fresh-Eyes Review Plugin

Overview

This plugin enforces a mandatory final quality gate before code ships. It catches security vulnerabilities, logic errors, and bugs that slip through despite passing tests.

Skill Included

fresh-eyes-review

Trigger keywords: commit, PR, done, complete, finished, ship, deploy, push

When to use:

  • ALWAYS before git commit
  • ALWAYS before creating pull request
  • ALWAYS before declaring work complete
  • After all tests pass but before shipping

What it does:

  1. Announces "Starting fresh-eyes review of [N] files. This will take 2-5 minutes."
  2. Systematically checks for:
    • Security vulnerabilities (SQL injection, XSS, path traversal, command injection)
    • Logic errors (off-by-one, race conditions, null handling)
    • Business rule bugs (calculations match requirements?)
    • Input validation (type, range, format checks complete?)
    • Performance issues (N+1 queries, unbounded loops)
  3. Fixes issues immediately
  4. Re-runs tests after corrections
  5. Announces results: "Fresh-eyes complete. [N] issues found and fixed."

Time commitment: 2-5 minutes

Core Principle

"NO COMMIT WITHOUT FRESH-EYES REVIEW FIRST"

This is non-negotiable. The discipline applies universally, even without explicit skill activation.

Key Distinctions

  • Testing - Validates expected behavior under controlled conditions
  • Code review - Examines patterns and quality during implementation
  • Fresh-eyes - Catches unexpected issues through deliberate re-reading with psychological distance

The skill emphasizes: "100% test coverage and passing scenarios" can coexist with "critical bugs" waiting discovery.

Resistance Patterns to Reject

The skill explicitly lists rationalizations to ignore:

  • "Tests are comprehensive"
  • "I'm confident it's correct"
  • "Partner is waiting"
  • "Production is blocked"
  • "Senior dev already approved"

These circumstances represent precisely when critical bugs escape into production.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
