Permanent Records & Public Actions Policy

CRITICAL: GitHub comments, PR/Issue modifications, and Git pushes leave permanent traces that cannot be fully erased (e.g., "comment deleted" logs). ALWAYS use notify_user (or AskUserQuestion) to get explicit confirmation BEFORE:
