A set of instructions for keeping a history of confirmed bugs, fixes, affected files, and recurring bug patterns in a Markdown report. Markdown is a plain-text format for structured documentation.

In plain words
What is it for?
Use it after a bug investigation to update totals, file risk summaries, patterns, and the list of recent or still-open bugs.
Why use it?
It prevents findings from being forgotten and makes it easier to see which files and bug patterns have the most history.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00529 $0.00529
Opus 5 $0.00264 $0.00264
Sonnet 5 $0.00106 $0.00106
Haiku 4.5 $0.00053 $0.00053

Measured 4d ago against content hash ca119c0d28ff, method: parsed. Prices are Anthropic first-party input rates as of 2026-09-12, from the pricing page.

Security

Grade A, and why

buggy buggy-bug-trends.md scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 4d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Buggy — Bug Trend Tracking

After every Buggy investigation that finds a confirmed bug, update the bug trend report at .debugger/bug-report.md.

When to Update

After any buggy_investigate call returns status: "confirmed_and_repaired" or status: "confirmed_no_repair".

Report Format

Maintain .debugger/bug-report.md with this structure:

# Bug Trend Report

Last updated: [timestamp]

## Recent Bugs

### [date] — calculateSubtotal (src/cart.ts)
- **Type:** Negative result from unclamped subtraction
- **Trigger:** `{price: 0.5, quantity: 15}`
- **Status:** Fixed ✓

Rules

  1. Only add entries for PROVEN bugs (with certificates), not speculative issues
  2. Mark bugs as "Fixed ✓" when an approved patch is applied
  3. Update the risk heatmap colors based on open bug count:
    • 0 open = 🟢 Clean
    • 1 open = 🟡 Medium
    • 2+ open = 🔴 High
  4. Keep the "Recent Bugs" section to last 20 entries
  5. Update "Bug Patterns" by grouping similar root causes
Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
