md-vision-mcp AGENTS.md is an instructions file for Codex, OpenCode from japlete/md-vision-mcp. It costs 630 tokens per session, scanned A, original, MIT.

Repository instructions for japlete/md-vision-mcp, a Node and TypeScript server that reads Markdown with embedded images and indexes headings across files or folders. They describe the project layout, technology choices, current status, and coding rules.

In plain words
What is it for?
Use them when working in this repository to locate server, tool, Markdown, image, and input/output code, follow its TypeScript and module rules, and run the test suite after changes.
Why use it?
They give an agent the project-specific facts needed to make safe changes and avoid breaking its conventions. They also identify the relevant tests and the command used to run them.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00630 $0.00630
Opus 5 $0.00315 $0.00315
Sonnet 5 $0.00126 $0.00126
Haiku 4.5 $0.00063 $0.00063

Measured 6d ago against content hash d3dab067d2a8, method: parsed. Prices are Anthropic first-party input rates as of 2026-09-06, from the pricing page.

Security

Grade A, and why

md-vision-mcp AGENTS.md scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 6d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

MD Vision MCP server

stdio MCP server: read markdown with inlined images (read_md_with_images) and index headings across files or folders (index_md). For agentic RAG and documentation workflows.

Status

Implemented. Entry: src/server.tsdist/server.js (md-vision bin). Tests: test/**/*.test.ts.

Layout

Path Role
src/server.ts MCP bootstrap, tool registration, stdio transport
src/tools/ Tool handlers and zod input schemas
src/markdown/ remark AST: parse, sections, line ranges, index rows
src/images/ Image load, MIME sniff, sharp/resvg → PNG
src/io/ URI fetch, path/domain allowlists
src/mcp/result.ts MCP content / error helpers

User-facing install and examples: README.md. MCP authoring patterns: .agents/skills/build-mcp-server/.

Stack

Node 20+, TypeScript, ESM (tsup). MCP SDK + zod. unified/remark (GFM, frontmatter). sharp, resvg, file-type, fast-glob. vitest, nock, tmp-promise.

Rules

  • Run npm test after code changes.
  • Use .js extensions in relative imports. No CommonJS.
  • stdio: never log to stdout; use console.error only.
  • .npmrc enforces 14-day release age for npm deps.

Tools (contract)

read_md_with_imagesuri (local path or URL), optional section (exact heading text, e.g. ## Intro), optional line_range [start, end] (section when matched, else line_range), optional max_images (default 10, max 50). Returns text + image blocks; frontmatter preserved via remark-stringify.

index_mduri (file, folder, or URL). Per file: frontmatter + TSV (heading, line_start, n_images, char_count) in <file path="..." lines=X chars=Y>. Headings inside fenced code are skipped (AST, not regex).

Images: markdown ![](...) and HTML <img src="...">; resolve relative to doc base; MIME via file-type then mime-types; raster → sharp PNG, SVG → resvg PNG.

Access

CLI: --allow-path and --allow-domain (both required, repeatable or =value). Server fails to start without at least one of each. --allow-domain=all allows any HTTP(S) host; --allow-domain=none disables URLs. Avoid bare * (shell glob).

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
