mcp-adr-analysis-server: Instructions file for Codex

AGENTS.md

mcp-adr-analysis-server AGENTS.md is an instructions file for Codex, OpenCode from tosin2013/mcp-adr-analysis-server. It costs 871 tokens per session, scanned A, original, MIT.

Repository instructions for coding agents working on a project where a human approves each change through a GitHub issue. An AGENTS.md file is a set of rules an agent reads before editing code.

In plain words
What is it for?
Use it to guide implementation from approved feature documents, GitHub issue rulings, architecture decisions, and project roadmaps.
Why use it?
It stops agents from starting work without an approved issue and defines which specifications, decisions, and memory files they must follow.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00871 $0.00871
Opus 5.5 $0.00348 $0.00348
Sonnet 5.5 $0.00174 $0.00174
Haiku 4.5 $0.00087 $0.00087

Security

Grade A, and why

mcp-adr-analysis-server AGENTS.md scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 8d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

The opening of the file, as written. The whole thing — 60 lines — stays where its author put it; the contents beside it link to each section on GitHub.

AGENTS.md

Instructions for coding agents that build changes in this repository (OpenCode, Claude Code in a builder loop, or any harness that reads AGENTS.md). Project commands, architecture and conventions are in CLAUDE.md; read it first.

This repo follows the Invoice Exception Desk PLAYBOOK: a human rules on every change in a GitHub issue, a builder implements exactly that ruling, checks the builder never saw grade it, and a human merges.

When these rules apply

Only when you are working an issue labelled agent-ready (an "Agent ruling" issue). That issue is the ruling: it answers every question you would otherwise guess. Without the label, don't start.

Which row gets built next is the operator's pick from ROADMAP.md ("How the next row is picked"), never the builder's.

Builder rules

Inputs: .specify/memory/constitution.md, the ruling issue, and specs/<NNN-feature>/spec.md, plan.md and tasks.md. A human approved them. Read them, every ADR listed in the spec's Governing Decisions section (in docs/adrs/), and ralph-memory.md if it exists, before you change anything. If a task would contradict an Accepted ADR, that's a BLOCKED stop (rule 6).

  1. Work only in the feature folder you were given.
  2. Take the first user story (phase) in tasks.md that still has unchecked tasks. Implement its tasks in order, dependencies first, non-[P] tasks before [P] ones. Work on that one story only.
  3. Run the tests: npm run typecheck && npm test. If they fail, fix them within this story, at most 3 tries. Tick only the tasks whose tests pass; if none pass, tick nothing, commit nothing, end your turn.
  4. Tick the completed tasks [x] in tasks.md, update ralph-memory.md and progress.md, and make one commit with the change, tasks.md, ralph-memory.md and progress.md. Then end your turn. Don't start the next story.
  5. Never run /speckit.specify, /speckit.clarify, /speckit.plan or /speckit.tasks during a builder loop. Never edit spec.md, plan.md or the constitution. Don't add, reword or delete tasks.
  6. If a task is unclear, conflicts with the spec, plan or ruling, or needs something that isn't done: don't guess and don't skip ahead. Append BLOCKED: <task ID>: <question> to progress.md, commit nothing, and end your turn.
  7. Print <promise>COMPLETE</promise> on its own line only when every task is [x], the tests pass and git status --short --untracked-files=all is empty.
  8. Don't report sizes or counts. CI (Acceptance Grader) does that.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
