  • Never rewrite, delete, or reorder existing ADR entries.
  • Never modify existing ADR text in place (including ADR 19).
  • Additions must be append-only:
    • add a new ADR, or
    • append a dated addendum section without altering prior content.
  • If a request conflicts with this policy, preserve existing ADR content and suggest an append-only alternative.
