Compile the session into a readable markdown document:
- A short title/summary line at the top
- Chronological sections for major pieces of work, each with what was asked and what was done (paraphrase the conversation, don't just dump raw turns)
- Any code changes, referenced by file path with a short description of the change rather than pasting full diffs unless the user wants that level of detail
- Open questions or follow-ups left at the end
