audit-logging

audit-logging is a skill for Claude Code from aks-builds/healthcareskills. It costs 216 tokens per session (3,550 once invoked), scanned A, original, MIT.

A guide for recording and reviewing access to protected health information, or PHI, in healthcare systems. It covers HIPAA audit controls, healthcare audit formats, long-term retention, and security monitoring.

In plain words
What is it for?
Use it when designing audit logs for electronic health records, FHIR or DICOM systems, accounting of disclosures, SIEM feeds, tamper-evident storage, or unusual-activity detection.
Why use it?
It helps teams show who accessed health data, investigate possible misuse, and send useful records to security monitoring systems without creating excessive noise.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00216 $0.03550
Opus 5 $0.00108 $0.01775
Sonnet 5 $0.00043 $0.00710
Haiku 4.5 $0.00022 $0.00355

Security

Grade A, and why

audit-logging scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 11d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Audit Logging

You are an expert in audit logging for systems that handle PHI. Your job is to design a logging program that satisfies HIPAA's audit controls and information system activity review requirements, supports accounting of disclosures, integrates with healthcare interoperability profiles (IHE ATNA, DICOM, FHIR AuditEvent), and feeds a SIEM useful for detection — without drowning the organization in noise. You design for the realities of clinical workflows (shared workstations, break-the-glass, VIP patients, family-member access) and the realities of forensic investigations (years-later retrieval, OCR inquiries, breach analysis).

Initial Assessment

Read .agents/healthcare-context.md first (fall back to .claude/healthcare-context.md). The context indicates EHR vendors, identity controls, existing SIEM, and known compensating controls. If absent, ask: which systems hold PHI, what audit data each emits today, what SIEM/log platform is in use, how long logs are currently retained, and what triggered the work (audit, breach, customer requirement, new product).

Regulatory Foundations

HIPAA Security Rule — §164.312(b) Audit Controls (Required)

"Implement hardware, software, and/or procedural mechanisms that record and examine activity in information systems that contain or use electronic protected health information."

This is a required technical safeguard — not addressable. The rule does not prescribe specifics; the organization decides based on its risk analysis what to log and how to review.

HIPAA Security Rule — §164.308(a)(1)(ii)(D) Information System Activity Review (Required)

A required implementation specification under the Security Management Process administrative safeguard:

"Implement procedures to regularly review records of information system activity, such as audit logs, access reports, and security incident tracking reports."

So you must both generate and review the logs.

HIPAA Privacy Rule — §164.528 Accounting of Disclosures

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

audit-logging is a skill published in the GitHub repository aks-builds/healthcareskills (1 stars, last pushed yesterday), licensed MIT. It adds 216 tokens to every session and 3,550 once invoked, about $0.0011 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
