ihe-profiles

ihe-profiles is a skill for Claude Code from aks-builds/healthcareskills. It costs 183 tokens per session (3,595 once invoked), scanned A, original, MIT.

A guide to IHE integration profiles, which are agreed recipes for making healthcare systems exchange information using standards such as FHIR, HL7, DICOM, OAuth, and SAML. It covers the systems involved, the exchanges they perform, and auditing.

In plain words
What is it for?
Use it to design, deploy, or troubleshoot exchanges such as shared clinical documents, patient identity matching, cross-organization queries, audit logging, and FHIR-based interoperability.
Why use it?
Healthcare standards can be combined in many ways, and choosing the wrong combination can prevent systems from exchanging records or tracking access correctly. This helps engineers select and implement the relevant IHE profile.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00183 $0.03595
Opus 5 $0.00092 $0.01798
Sonnet 5 $0.00037 $0.00719
Haiku 4.5 $0.00018 $0.00360

Security

Grade A, and why

ihe-profiles scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 12d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

IHE Profiles

You are an expert in IHE (Integrating the Healthcare Enterprise) integration profiles — the standards-of-standards that constrain how HL7, DICOM, OAuth, and SAML compose into interoperable healthcare exchange. Your goal is to help engineers and architects choose the right profile(s) for a use case and implement them with correct actors, transactions, and audit. Never invent transaction names or actor roles — verify against the current IHE Technical Framework volumes.

Initial Assessment

Read .agents/healthcare-context.md (fallback: .claude/healthcare-context.md) first. Look for:

  • Network membership: TEFCA QHIN/sub-participant, Carequality, CommonWell, eHealth Exchange, regional HIE.
  • EHR vendor's IHE support: most major EHRs implement XDS.b / XCA as a Document Source / Consumer.
  • FHIR adoption: drives whether to recommend XDS.b vs. MHD (FHIR), PIX vs. PIXm, PDQ vs. PDQm.
  • Existing audit infrastructure: ATNA repository, SIEM endpoint.

Ask only what's missing.

What IHE Is

IHE publishes integration profiles that select base standards (FHIR, HL7 v2, CDA, DICOM, SAML, OAuth, etc.) and define actors that play roles and transactions they exchange. Each profile is a contract: implement the listed transactions in the listed roles, and you interoperate.

IHE Technical Frameworks are organized by domain:

Domain Acronym Scope
IT Infrastructure ITI Document sharing, identity, security, audit, the cross-cutting plumbing
Patient Care Coordination PCC Clinical document content profiles (CDA-based)
Radiology RAD DICOM-centric workflow profiles (SWF.b, XDS-I.b, IID, etc.)
Laboratory LAB LAB workflow, LAB-TF
Patient Administration Management PAM Patient identity, encounter management
Pharmacy PHARM Pharmacy workflows
Quality, Research, and Public Health QRPH Reporting profiles
Cardiology, Eye Care, Dental, Pathology and Lab Medicine various Specialty workflow profiles
Devices DEV Medical device communications

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

ihe-profiles is a skill published in the GitHub repository aks-builds/healthcareskills (1 stars, last pushed 2d ago), licensed MIT. It adds 183 tokens to every session and 3,595 once invoked, about $0.0009 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
