pii-redaction

pii-redaction is a skill for Claude Code, Codex from akshaykanni22/medbill-scanner. It costs 0 tokens per session (2,941 once invoked), scanned A, original, MIT.

A guide for adding a service that removes personally identifying information from text. Personally identifying information includes details such as names, patient data, financial records, and other information that can identify someone.

In plain words
What is it for?
Use it to design PII redaction for medical bills, insurance forms, financial documents, HR records, and other user-submitted text.
Why use it?
It helps prevent sensitive text from being sent to external services, saved in databases, or written to logs before it has been cleaned.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00000 $0.02941
Opus 5 $0.00000 $0.01470
Sonnet 5 $0.00000 $0.00588
Haiku 4.5 $0.00000 $0.00294

Security

Grade A, and why

pii-redaction scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 11d ago.

The scan reads SKILL.md. This mod also ships 2 executable files (pii_redactor.py, test_pii_redactor.py), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

SKILL: PII Redaction

What This Skill Does

Teaches Claude Code how to write a production-safe PII redaction service for any project handling sensitive personal data — medical bills, insurance forms, financial documents, HR records, etc.

When To Use This Skill

Invoke this skill when a project needs to strip personally identifiable information from text before that text is sent to any external API, stored in a database, or logged anywhere.

Trigger phrases:

  • "add PII redaction"
  • "strip patient data before the API call"
  • "redact sensitive information"
  • "implement PHI removal"

Core Principles (read before writing any code)

1. Redact early, redact once PII redaction must happen as the FIRST step in any pipeline that handles user-submitted text. Never pass raw text to downstream services and rely on them to handle it safely.

2. Regex over ML models for this task Do not use spaCy, Presidio, or any NLP model for basic PII redaction unless the project explicitly requires entity-level classification. Reasons:

  • Regex is auditable — you can read exactly what it catches
  • ML models have false negatives that are invisible and unpredictable
  • Regex has zero cold start time, no model download, no GPU needed
  • For structured PII (SSN, phone, email, dates) regex is more reliable than NLP anyway

3. Replace don't delete Always replace PII with a labeled placeholder like [SSN], [NAME], [DATE_OF_BIRTH]. Deleting makes the redacted text lose structure and confuses downstream LLM reasoning. Labeled placeholders let the LLM understand what was there without seeing the actual value.

4. Log redaction counts, never redacted values Log how many items were redacted per category for debugging. Never log the original PII values even in debug mode.

5. Validate redaction before sending to any API After redacting, run a second-pass check to catch any PII that slipped through. Fail loudly if the second pass finds anything.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

pii-redaction is a skill published in the GitHub repository akshaykanni22/medbill-scanner (3 stars, last pushed 3mo ago), licensed MIT. It costs nothing until one of its globs matches a file; then it loads 2,941 tokens. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
