handoff-docs

handoff-docs is a skill for Claude Code, Codex from alphaelements/handoff-mcp. It costs 125 tokens per session (6,473 once invoked), scanned A, original, MIT.

A document-management workflow for saving, reading, searching, importing, and linking structured project documents such as specifications, designs, guides, and notes.

In plain words
What is it for?
Use it to register specifications, import Markdown document collections, find sections, follow document relationships, and connect documents to tasks.
Why use it?
It keeps important project knowledge available across sessions and makes related documents and tasks easier to trace.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5 $0.00125 $0.06473
Opus 5 $0.00063 $0.03236
Sonnet 5 $0.00025 $0.01295
Haiku 4.5 $0.00013 $0.00647

Measured 3d ago against content hash ac057d8b9114, method: parsed. Prices are Anthropic first-party input rates as of 2026-08-30, from the pricing page.

Security

Grade A, and why

handoff-docs scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 3d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Handoff Docs Skill

When to use

  • The user asks to save a spec, design doc, ADR, guide, or note so it survives across sessions
  • The user wants to import an existing pile of Markdown files (e.g. a wiki/ or specs/ directory) into structured document management
  • You need to read a specific section of a large document without pulling the whole thing into context
  • You need to trace how documents relate to each other (parent/child hierarchy, or semantic links like "this design implements that spec")
  • A document should be linked to one or more tasks so it surfaces automatically while that task is being worked on

If the knowledge is a short, standalone lesson/rule/convention/gotcha (< 1 page, no internal sections), use handoff-memory instead — see "Memory vs Documents" in skills/handoff-memory/SKILL.md for the boundary.

Development Flow Integration

Document management is NOT just for explicit user requests. It activates automatically during the standard development cycle:

When writing a spec or design

After writing/updating a specification or finishing a /design-review session (wiki/ or tmp/), immediately:

  1. Start from a template (see "Templates" below) instead of a blank document.
  2. handoff_doc_save(slug="unique-spec-slug", title=..., body=..., doc_type="spec", task_ids=[...])
  3. handoff_doc_verify(doc_id=..., action="generate") to create the verification matrix

When starting a task

Before implementation, fetch related specs:

  • handoff_doc_query(task_id="<task-id>") — surfaces linked documents automatically
  • Review the verification matrix: handoff_doc_verify_status(doc_id=...)

When implementation is complete

Mark verified sections:

  • handoff_doc_verify(doc_id=..., action="check", fragment_seq=N) for each completed section
  • handoff_doc_verify(doc_id=..., action="set_refs", fragment_seq=N, impl_refs=[...]) to record implementation locations

When reviewing

Check readiness:

  • handoff_task_checklist(task_id=..., action="view") — combined readiness view

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
