kamehadb: Skill for Claude Code

.agents/skills/document-wiki/SKILL.md

document-wiki is a skill for Claude Code, Codex from asta-nguyen/kamehadb. It costs 30 tokens per session (2,620 once invoked), scanned A, original, Apache-2.0.

A documentation procedure for creating or refreshing a repository wiki from source code that has been checked during the current run.

In plain words
What is it for?
Use it to document routes, files, modules, and selected features in a repository's agent-readable wiki.
Why use it?
It prevents unverified documentation from being presented as current. It distinguishes incomplete verification from confirmed gaps in the code documentation.

Written for no agent in particular: nothing here depends on one. Also seen: installed under .agents/ (shared by several agents); mentions AGENTS.md.

This is asta-nguyen/kamehadb's own configuration. It tells Claude Code and Codex how to work on kamehadb itself, so it is not a mod to install elsewhere. Copy it as a starting point and replace the rules that are about this project. Everything kamehadb configures →

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00030 $0.02620
Opus 5.5 $0.00012 $0.01048
Sonnet 5.5 $0.00006 $0.00524
Haiku 4.5 $0.00003 $0.00262

Security

Grade A, and why

document-wiki scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 19d ago.

The scan reads SKILL.md. This mod also ships 1 executable file (scripts/validate-llm-wiki.mjs), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 187 lines — stays where its author put it; the contents beside it link to each section on GitHub.

Document Wiki

Create or refresh the target repository's docs/llm/ from verified source code. This is the one-stop wiki workflow; do not hand the user off elsewhere.

Completion contract

An inventory of routes, files, or modules is baseline coverage, not deep feature documentation. A selected feature is complete only when its page explains current business behavior and its material claims are traceable to source or tests read in the current run. Mark [ ] an undocumented feature and [x] a feature verified against that evidence. Use [~] for either a verification limit (checks remain incomplete or required evidence is inaccessible; the page may be correct, so it is not a refresh candidate) or a confirmed content gap (completed checks prove a material omission or a claim that contradicts source; a refresh candidate). State which one and why; they are not equivalent. Never call unverified coverage current.

Task updates

When implementation calls this skill for an active task, use this procedure instead of the whole-wiki Workflow below. The task's scope selects its affected behavior automatically; no extra page-selection approval is needed. Standalone wiki requests use the Workflow and its selection gate.

  1. If docs/llm/ is absent, return not-applicable without setup or wiki creation. Otherwise apply Workflow step 1's instruction preflight, without its missing-wiki setup, and read the index and relevant pages. Use approved scope, the diff, and the current impact map/caller trace to select changed behavior, including callers outside the diff. Inventory or document no neighboring feature and create no repository-wide overview.
  2. Merge any task delta with ## Merge a task delta before other content edits, preserving its exact after-merge blocks. If a delta block itself needs correction, reconcile the delta and wiki together using source and approved decisions; unresolved conflicts stop the update. A merge marker alone never proves the merge is correct.
  3. Read current source and relevant tests and use the evidence matrix and page template for the selected behavior. Refresh stale content and add missing task behavior to the smallest relevant page. If its domain has no page, create a scoped page and register its prefix per Workflow steps 7-8; source-established domain boundaries need no approval. Reuse existing IDs/prefixes, preserve unrelated content, and report unrelated gaps as optional suggestions. A new page documents this task's behavior, not its module's every feature.
  4. Ask only for unresolved behavior, conflicting requirements/source, domain boundaries not established by evidence, missing access, or work beyond the task's scope. Missing evidence for required task behavior keeps the task incomplete; do not invent it or mark it complete with an open question.
  5. Update Sources, index links, and retired IDs as applicable and run ## Verify the wiki. Report any legacy-link migration conflict or scope issue. Verify each changed claim against source and tests: a validator pass or an Evidence path alone does not prove behavior.
  6. Return changed pages, covered behavior, checks, and blockers to the caller. Never archive, mark task progress, or change approval here; implementation and final review own those actions.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
