failure-codifier

A read-only tool that finds repeated failure patterns in orchestration logs and a Judgment Ledger, then proposes structured failure rules with confidence levels. It does not automatically change the approved source-of-truth files.

In plain words
What is it for?
Use it to analyse repeated failures, produce dry-run failure-rule proposals, assign confidence based on occurrences, and suggest whether a rule belongs in patterns.md or decisions.md.
Why use it?
It helps turn recurring mistakes into reviewable lessons without silently changing team rules. Human approval remains required before any proposed rule is adopted.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5 $0.00092 $0.00711
Opus 5 $0.00046 $0.00356
Sonnet 5 $0.00018 $0.00142
Haiku 4.5 $0.00009 $0.00071

Measured 3d ago against content hash b57608a39c7c, method: parsed. Prices are Anthropic first-party input rates as of 2026-08-30, from the pricing page.

Security

Grade A, and why

failure-codifier scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 3d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Failure Codifier

breezing orchestration ledger + Judgment Ledger から再現失敗を read-only で抽出し、failure-rule.v1 候補を confidence score 付きで提案します。

核心契約

  • human-approval-required: codifier は dry-run 提案のみ。patterns.md / decisions.md への自動昇格は構造的に禁止。
  • Confidence 閾値: occurrence count ≥ 3 → mediumcount ≥ 5 → highgo/internal/failurecodifier/confidence.go)。
  • 昇格先 heuristic: proposed_ssot_target フィールドで patterns.md または decisions.md提案するのみ

使い方

Dry-run 提案(推奨)

./scripts/failure-codifier-propose.sh --dry-run

stdout は JSON 配列(failure-rule.v1 候補)。SSOT ファイルには一切書き込みません。

Go テスト

cd go && go test ./internal/failurecodifier/... -count=1

Files

What ships with it

1 file beside SKILL.md in the same directory: the scripts, references and assets a skill reads on demand. Not counted in the per-session cost; read them before you install if any of them is executable.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

failure-codifier is a skill published in the GitHub repository Chachamaru127/claude-code-harness (3,079 stars, last pushed 2d ago), licensed MIT. It adds 92 tokens to every session and 711 once invoked, about $0.0005 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-30.
