reflect

reflect is a skill for Claude Code, Codex from chhoumann/t3-pstack. It costs 36 tokens per session (1,554 once invoked), scanned A, original, MIT.

A reflection workflow that reviews the current conversation for reusable lessons and turns them into concrete edits to existing skills. It uses three parallel reviewers to surface those lessons.

In plain words
What is it for?
Use it when asked to reflect on a session and update an existing skill based on a durable learning.
Why use it?
It helps improve the instructions used for future work instead of leaving useful discoveries in a single conversation.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00036 $0.01554
Opus 5.5 $0.00014 $0.00622
Sonnet 5.5 $0.00007 $0.00311
Haiku 4.5 $0.00004 $0.00155

Security

Grade A, and why

reflect scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured yesterday.

The scan reads SKILL.md. This mod also ships 1 executable file (scripts/find-transcript.mjs), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Reflect

Mine the current conversation for durable learnings, then route them into skill edits.

Invoke when the user says "reflect" or "/reflect". Skip when the conversation is trivial, off-topic, or already covered by an existing skill the parent followed correctly. One-offs are not learnings.

1. Locate the active transcript

Before fanning out, find this conversation's own transcript where this thread's host keeps it. Do not search other projects' transcripts. That crosses workspace boundaries and reads private chats from unrelated projects.

On Claude Code, run the finder with this session's ID, this project's transcripts directory, and a fragment of the conversation's opening user prompt:

node "<plugin root>/skills/reflect/scripts/find-transcript.mjs" --session "$CLAUDE_CODE_SESSION_ID" "${CLAUDE_CONFIG_DIR:-$HOME/.claude}/projects/<encoded-cwd>" "<opening prompt fragment>"

<plugin root> is the directory that holds this plugin's skills/, and <encoded-cwd> is the working directory with each / and . turned into -. With the session ID set, the finder prints this session's own transcript, and exits 1 when it does not exist, because a scan could return a sibling session that shares the opening prompt. Without it, the finder scans the flat, nested, and subagent layouts newest first and prints the first transcript whose opening typed prompt carries the fragment. Do not reimplement the scan by hand.

On Codex, pass this thread's ID, which Codex exports as CODEX_THREAD_ID, to the resolver, and never search by chat title or time:

"<plugin root>/skills/poteto-mode/scripts/resolve-codex-transcript" "$CODEX_THREAD_ID"

On success it exits 0 and prints JSON whose path is the transcript. If either command exits non-zero, pass a tight digest of the session instead.

2. Spawn three reviewers in parallel

One response, three delegate_task calls with mode: "async" and full tool access. Reviewers need MCP access to look up the tickets, chat threads, and observability traces the transcript references.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

reflect is a skill published in the GitHub repository chhoumann/t3-pstack (190 stars, last pushed yesterday), licensed MIT. It adds 36 tokens to every session and 1,554 once invoked, about $0.0001 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-10-13.
