The document-level x-express-recon.schemasArePlaceholders: true means some
generated surface may remain under-specified. Low/medium-confidence fragments
carry x-express-recon-unrefined: true; high-confidence static Fastify,
Zod/Joi, express-validator, or NestJS validator/Swagger evidence does not.
Your job is to verify that evidence and refine only what remains ungrounded.
