Tie every completion claim to exact evidence

For each claim such as done, fixed, working, verified, merged, deployed, released, or secure, locate matching evidence from the pull request head: tests, exact-head checks, artifacts, screenshots, runtime receipts, commit state, or deployment state. A green unrelated check is not proof. Report missing or stale evidence as Unverified rather than inferring success. Include severity and confidence for defects, and distinguish Source-backed, Locally verified, Verified, Assumption, and Blocked claims.
