setup

setup is a skill for Claude Code from cyanheads/medical-codes-mcp-server. It costs 58 tokens per session (2,458 once invoked), scanned A, a copy of setup, Apache-2.0.

A setup and orientation guide for an MCP server created with the cyanheads framework. It explains the generated files, project layout, conventions, and how skills are synchronized.

In plain words
What is it for?
Use it to learn the roles of CLAUDE.md and AGENTS.md, locate server definitions, read framework documentation, and choose next development checks.
Why use it?
It helps a developer or coding agent understand an unfamiliar server after initialization or when joining an existing project.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00058 $0.02458
Opus 5 $0.00029 $0.01229
Sonnet 5 $0.00012 $0.00492
Haiku 4.5 $0.00006 $0.00246

Security

Grade A, and why

setup scanned grade A with 1 finding against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 6d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Asks the agent to reveal its instructionslowSystem prompt leakage

Directions to print, repeat or translate the system prompt extract configuration the operator did not intend to expose.

1. **Clean up what you don't need.** If your server has no prompts, delete the echo prompt and its registration in `src/index.ts`. Same for resources, or the app-tool pair if you're not targeting UI-capable clients.

Downgraded: this mod is about security review, or the phrase is quoted, so it is likely naming the pattern rather than instructing it.

Context

This skill assumes bunx @cyanheads/mcp-ts-core init [name] has already run. The CLI created the project's CLAUDE.md and AGENTS.md for different agents, copied external skills to skills/, and scaffolded the directory structure with echo definitions as starting points. This skill covers what was created and what to do next.

Agent Protocol File

The init CLI generates both CLAUDE.md and AGENTS.md with identical content — CLAUDE.md is read by Claude Code, AGENTS.md by Codex, Cursor, Windsurf, and other agents. Keep both. Shipping both keeps the project agent-agnostic, and they're cheap to hold in sync: edit one, then cp CLAUDE.md AGENTS.md (the framework keeps its own pair byte-identical the same way, enforced by check-docs-sync). Only delete one if you're certain the project will never be opened by the other family of agents.

For the full framework docs, read node_modules/@cyanheads/mcp-ts-core/CLAUDE.md (or its identical twin AGENTS.md) once per session. It contains the exports catalog, tool/resource/prompt contracts, error codes, context API, and common import patterns.

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

setup is a skill published in the GitHub repository cyanheads/medical-codes-mcp-server (2 stars, last pushed 12d ago), licensed Apache-2.0. It adds 58 tokens to every session and 2,458 once invoked, about $0.0003 per session on Opus 5. A static security scan graded it A with 1 finding (asks the agent to reveal its instructions). It is 100% identical to setup, differing in 16 lines, and is treated as a copy.
