incident-diagnosis

incident-diagnosis is a skill for Claude Code, Codex from dayfinggg/openai-codex-agent-skills. It costs 39 tokens per session (373 once invoked), scanned A, original, MIT.

An incident-investigation guide for application errors, slowdowns, and outages. It builds a time-limited explanation from logs, measurements, traces, and recent changes.

In plain words
What is it for?
Use it to investigate backend or web incidents, compare affected and unaffected requests, test possible causes, and decide when a specialist investigation is needed.
Why use it?
It prevents confusing correlation with cause and helps separate application failures from dependency waits, connection problems, resource limits, and rollout effects.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00039 $0.00373
Opus 5.5 $0.00016 $0.00149
Sonnet 5.5 $0.00008 $0.00075
Haiku 4.5 $0.00004 $0.00037

Security

Grade A, and why

incident-diagnosis scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured today.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Find the cause of the reported incident with the least expensive reliable evidence.

  1. Establish the affected service, operation, environment, user-visible symptom, and fixed time range. Reuse supplied IDs, alerts, and known changes. Alert firing time is not automatically failure onset.
  2. Confirm datasource and environment identity before querying. Begin with the strongest supplied signal rather than surveying every dashboard or service.
  3. Build a timeline from evidence. Separate connection failures, application exceptions, dependency waits, resource saturation, and rollout changes. A temporal correlation alone does not prove causation.
  4. Form ranked hypotheses that can be weakened as well as supported. Choose a bounded check for each material uncertainty. Inspect counterevidence before making a cause claim.
  5. Compare equivalent request populations, parameters, and deployments. Distinguish missing data, inaccessible data, and an actual measured zero. Account for retention, sampling, and clock differences.
  6. Use php-fpm-opcache-diagnostics, database-query-diagnosis, or a relevant installed runtime specialist when the evidence points there. Keep investigation read-only until an authorized correction is established.
  7. Verify the original symptom after a correction and check the relevant regression signal. A restarted service or successful health endpoint does not by itself prove the failing operation is repaired.

Do not repeatedly restart components, raise limits, clear shared caches, or disable checks to make symptoms disappear. Ask only for a missing fact or access that materially blocks diagnosis and cannot be obtained.

Finish with verified observations, established cause or remaining uncertainty, actual corrective action, and the evidence for recovery. Keep the report in chat unless a file is requested.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

incident-diagnosis is a skill published in the GitHub repository dayfinggg/openai-codex-agent-skills (8 stars, last pushed today), licensed MIT. It adds 39 tokens to every session and 373 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-10-30.
