documentation-audit

documentation-audit is a skill for Claude Code, Codex from DojoGenesis/mcp. It costs 101 tokens per session (2,600 once invoked), scanned A, original, MIT.

A systematic check that compares project documentation with the code it describes.

In plain words
What is it for?
Use it to classify documentation as accurate, outdated, missing, or unused, then identify corrections.
Why use it?
It finds outdated instructions, missing information, broken links, and documents that no longer match the project.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5 $0.00101 $0.02600
Opus 5 $0.00051 $0.01300
Sonnet 5 $0.00020 $0.00520
Haiku 4.5 $0.00010 $0.00260

Measured 3d ago against content hash 0d531e0ae537, method: parsed. Prices are Anthropic first-party input rates as of 2026-08-30, from the pricing page.

Security

Grade A, and why

documentation-audit scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 3d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Documentation Audit Skill

Version: 1.0 Created: 2026-02-11 Purpose: Provide a structured, repeatable process for auditing project documentation, combating documentation drift, and ensuring all documents remain a reliable source of truth.

I. The Philosophy: Tending the Garden of Knowledge

Project documentation is a garden. When tended with care, it is a source of clarity, guidance, and shared understanding. When neglected, it becomes overgrown with outdated information, broken links, and misleading instructions -- a phenomenon known as documentation drift. This drift erodes trust and creates confusion.

The documentation audit is the practice of tending the garden. It is a recurring ritual: walking through documentation, pulling the weeds of inaccuracy, and pruning the branches of irrelevance. It is an act of stewardship, ensuring that the shared garden of knowledge remains a welcoming and reliable resource.

Documentation drift is insidious because it happens silently. Code changes daily; documentation changes monthly (if you're lucky). The longer the gap between code changes and doc updates, the wider the drift. Regular audits catch drift early, when corrections are small and cheap.

II. When to Use This Skill

  • After a major release: To ensure all documentation reflects new features and changes.
  • Before onboarding a new contributor or agent: To ensure they receive accurate information.
  • As a scheduled, recurring task: (e.g., monthly) to maintain regular review cadence.
  • When documentation "feels" out of sync with the code.
  • After a refactor or architectural change: These are the highest-risk moments for drift.
  • As part of a broader health audit: Dimension 5 (Documentation) of the health audit can trigger a deep documentation audit.

When NOT to use: For writing new documentation from scratch (that's a different skill). For reviewing a single document in isolation (just read and fix it). For auditing code quality (use the health audit skill instead).

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

documentation-audit is a skill published in the GitHub repository DojoGenesis/mcp (0 stars, last pushed 1mo ago), licensed MIT. It adds 101 tokens to every session and 2,600 once invoked, about $0.0005 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
