fix-from-qa-report

fix-from-qa-report is a skill for Claude Code, Codex from Espenandreass1/agentslice. It costs 26 tokens per session (193 once invoked), scanned A, original, MIT.

A guided process for fixing problems that an independent quality check has identified as blockers. It limits the changes to the approved work and then prepares another independent check.

In plain words
What is it for?
Use it after QA, meaning a separate review that checks whether the software works as intended, reports a blocker. It helps apply the approved correction, run focused checks, and request a re-test.
Why use it?
It prevents fixes from quietly expanding the agreed product or scope. It also makes clear that correcting a problem does not replace a fresh quality check or release approval.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00026 $0.00193
Opus 5 $0.00013 $0.00097
Sonnet 5 $0.00005 $0.00039
Haiku 4.5 $0.00003 $0.00019

Security

Grade A, and why

fix-from-qa-report scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 3d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Fix From QA Report

Read

Verify the workspace and minimal preflight. Read the PR, changed files, coding rules, and the blocker evidence or exceptional QA report.

Guardrails

  • Fix only approved blockers inside PR scope. A new area, lane, or user outcome needs reclassification and approval.
  • Do not turn a fast-bug into a broader feature silently; CI limits remain active.
  • Keep sensitive safeguards intact and require independent re-test.

Steps

  1. State only the blocker and intended fix.
  2. Implement the narrow fix and run focused checks.
  3. Update PR live status and request independent QA re-test.

Output

Report changed files, one-line checks, residual risk, and re-test next action. Do not create a duplicate status document.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
