Getting it into your agent
One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00040 | $0.00597 |
| Opus 5 | $0.00020 | $0.00298 |
| Sonnet 5 | $0.00008 | $0.00119 |
| Haiku 4.5 | $0.00004 | $0.00060 |
Grade A, and why
mcp-surveys-cli scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 9d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
What it actually says
mcp-surveys-cli
Use the CLI plus this skill as the default setup. Secure mode keeps keys local in the CLI receipt and the hosted service stores only ciphertext.
Default hosted instance:
uvx --refresh-package mcp-surveys-cli mcp-surveys-cli schema
uvx --refresh-package mcp-surveys-cli mcp-surveys-cli template decision > survey.json
uvx --refresh-package mcp-surveys-cli mcp-surveys-cli create survey.json
uvx --refresh-package mcp-surveys-cli mcp-surveys-cli wait <survey_id> --format markdown
uvx --refresh-package mcp-surveys-cli mcp-surveys-cli answers <survey_id>
create is secure by default. It prints survey_id, public_url, result_token, receipt_path, and expiry data. Send only public_url to the human. Keep result_token and receipt_path private.
The CLI saves the E2EE receipt at ~/.config/mcp-surveys/receipts/<survey_id>.json unless MCP_SURVEYS_RECEIPT_DIR is set. Later answers, export, and wait can use just survey_id; pass --receipt /path/to/receipt.json if running from another machine/session.
Plaintext is not a fallback. Use it only when the user explicitly asks for a non-private/shareable survey and accepts that the server can read survey data:
uvx --refresh-package mcp-surveys-cli mcp-surveys-cli create survey.json --mode plaintext
npx mcp-surveys-cli create fails closed for secure mode; use the Python uvx CLI for E2EE. npx ... create --mode plaintext is explicit plaintext opt-in only.
The hosted API has a critical upgrade gate: stale clients and removed MCP integrations get HTTP 426 with CRITICAL UPGRADE REQUIRED. Current CLI commands send anonymous client/version/mode headers automatically so stats can show usage without content, keys, tokens, or IP addresses.
Use MCP_SURVEYS_BASE_URL or --base-url for another instance.
Prefer structured buttons, ranking, matching, scale, color_choice, and binary_tradeoff; use text only when the answer cannot fit those shapes.
Any http(s):// URL in title, description, prompt, option text, scale labels, or tradeoff theses renders as a clickable link on the survey page (opens in a new tab). Use this to give the human context — docs, screenshots, dashboards — without dumping it into the prompt itself.
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

mcp-surveys-cli is a skill published in the GitHub repository EzzySoft/mcp-surveys (2 stars, last pushed 1mo ago), licensed MIT. It adds 40 tokens to every session and 597 once invoked, about $0.0002 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
