错误处理
NOTION_AUTH_REQUIRED：告诉用户前往“设置 → 资源链接 → Notion”连接或重新连接，然后重试。NOTION_PERMISSION_DENIED：告诉用户在 Notion 中向该连接授予目标页面/数据库权限，或重新连接。NOTION_CAPABILITY_UNAVAILABLE/NOTION_REQUEST_FAILED：说明服务暂不可用并建议稍后重试；继续回答不依赖 Notion 的部分。NOTION_RESOURCE_NOT_SELECTED：说明该 ID 不在当前已挂载索引中，引导用户在资源链接中选择并同步。
Notion Read hook 局部失败不等于整个 Agent 对话失败。不得猜测页面内容、伪造成功或要求用户粘贴 token。
