hexgate: Skill for Claude Code

.claude/skills/write-adr/SKILL.md

write-adr is a skill for Claude Code from HexamindOrganisation/hexgate. It costs 78 tokens per session (1,107 once invoked), scanned A, original, MIT.

A tool for writing Architecture Decision Records, or ADRs: files that preserve important technical decisions and the reasons behind them. It follows the repository's existing format and numbering rules.

In plain words
What is it for?
Use it to create or update one decision record in docs/adr/, including its status, affected files, required rules, and detailed reasoning.
Why use it?
It removes the risk of durable design or security decisions being recorded inconsistently or losing their rationale over time. It also handles marking older decisions as superseded.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00078 $0.01107
Opus 5.5 $0.00031 $0.00443
Sonnet 5.5 $0.00016 $0.00221
Haiku 4.5 $0.00008 $0.00111

Security

Grade A, and why

write-adr scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 6d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 98 lines — stays where its author put it; the contents beside it link to each section on GitHub.

Write a hexgate ADR

ADRs live in docs/adr/, one decision per file, named R-<AREA>-<NNN>-<kebab-summary>.md (e.g. R-GUARD-002-guards-fail-closed.md). Read a couple of existing records first — they are the source of truth for the house style — then match them.

Terse on what, detailed on why. An agent reads these under a token budget and acts on them literally, so the decision must be unambiguous and the rationale must survive: six months later nobody remembers why, and without it someone "cleans up" the decision and reintroduces the bug it was avoiding. The rationale is the one part you must never compress.

Before writing

  • Pick the area and the next id. Existing areas: AGENT, AUTH, GUARD, POL, USAGE. Reuse one if the decision fits it; open a new area only for a genuinely new subsystem. Number is the next free integer in that area (ls docs/adr/).
  • One decision per file. If you can't state it in two sentences, it's probably two decisions — split it.
  • Superseding? Don't delete the old record. Set the old one's Status to Superseded by R-<AREA>-NNN (date) with a short "Why superseded" note, and set the new one's Status to supersedes …, so the chain resolves. See R-AGENT-001 → R-AGENT-002 for the pattern.

Required shape

Every record has these; keep the optional ones only when they earn their place.

  • Title — # R-<AREA>-NNN: <the decision as a short imperative phrase>.
  • Status — Accepted · <YYYY-MM-DD>, or Superseded by … / supersedes ….
  • Applies to — the exact file/dir globs the decision governs, e.g. `hexgate/guards/**`, `hexgate/adapters/**/tools.py`. This is what scopes the ADR: an agent loads it when touching those paths, so it is not always-on context. Be specific.
  • ## Decision — one or two imperative sentences, then MUST / MUST NOT bullets. Never "prefer" or "consider" — those let an agent do the opposite.
  • ## Why — the forces, the tradeoff, and what breaks if the rule is ignored. Spend words here. Name the concrete failure the decision avoids (an incident, a fail-open, a drift) if there is one.

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

write-adr is a skill published in the GitHub repository HexamindOrganisation/hexgate (23 stars, last pushed today), licensed MIT. It adds 78 tokens to every session and 1,107 once invoked, about $0.0003 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-24.
