Core principle
Load every medicine. Diagnose observable state. Activate only applicable treatments. Resolve treatment conflicts. Execute relentlessly. Verify sufficiently. Prevent semantic loopholes. Prove completion. Stop. One explicit command replaces manually selecting and invoking 20-30 individual AI-Psychiatry skills; it never replaces authorization, safety, or approval boundaries.

Type: meta-superpower. Tier: god-mode. Risk: high. Invocation: explicit-only. All the medicine composes every registered public skill plus never-stop, but must never invoke itself recursively and must never run conflicting interventions at the same instant.
Dynamic skill inventory
The authoritative inventory installed in this repository is .ai/manifests/skills.json (plugin_skills array); this skill does not hand-maintain a duplicate list that can go stale. It considers every currently registered public skill plus never-stop, and excludes only itself (all-the-medicine). install-framework is applicable only when the current task is installing, upgrading, auditing, or repairing the framework's packaging; for ordinary coding tasks it is NOT_APPLICABLE.
Deep judgment for each phase routes to .ai/rules/57-all-the-medicine.md (the composite runtime rule) and .ai/guides/all-the-medicine.md; individual controls route to their own .ai/rules/ entry through canonical_rules in the manifest. There is no separate compiled copy inside .ai/ — the manifest and rules are already the deterministic, installed source of truth for this tree.
Procedure
- Lock the primary objective and the finite Definition of Done. Resolve any conflicting instruction sources by priority (system/platform, user, repository, domain, AI-Psychiatry, skill, memory, temporary state) before anything else.
- Build the complete skill status map: every applicable skill gets exactly one status among
PENDING,CHECKED,ACTIVE,SATISFIED,NOT_APPLICABLE,BLOCKED_BY_HIGHER_PRIORITY_RULE. Track only observable state and concise decision evidence per skill — never private chain-of-thought. - Select the single highest-priority applicable control from the execution-phase order below. Do not run conflicting interventions simultaneously.
- Apply exactly one control, then take the productive action it implies and capture observable evidence of the result.
- Update the completion matrix (requirement, status, evidence, remaining action) from that evidence only.
- Reevaluate every skill's status against the new observable state.
- Continue: mandatory work remaining -> next control; recoverable failure -> recovery ladder (from
never-stop); hard gate with independent work left -> keep working independently; hard gate with none left -> request the minimum approval; Definition of Done proven -> final report and stop. - Never loop back into a control whose status is already
SATISFIEDwithout a state change that reopens it.
