postmortem-facilitation

postmortem-facilitation is a skill for Claude Code, Codex from joao-aires/agent-skills-public. It costs 42 tokens per session (775 once invoked), scanned A, original, MIT.

A structured incident-review skill for reconstructing what happened during a service failure and producing or checking a blameless postmortem. A postmortem is a record of an incident, its causes, impact, uncertainty, and follow-up actions.

In plain words
What is it for?
Use it to prepare focused interviews, build an evidence record, analyze causes and impact, challenge unsupported claims, and draft or review postmortems.
Why use it?
It replaces plausible guesses and blame with evidence from timelines, changes, messages, and system data, while making unanswered questions visible.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00042 $0.00775
Opus 5.5 $0.00017 $0.00310
Sonnet 5.5 $0.00008 $0.00155
Haiku 4.5 $0.00004 $0.00077

Security

Grade A, and why

postmortem-facilitation scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured yesterday.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Postmortem facilitation

Read composition guidance for optional expertise, fallback and scope.

Reconstruct how the system and response produced the observed impact. Separate the event that triggered the incident from the conditions that let it propagate, the signals that revealed it, and the actions that restored service. Preserve what responders knew at the time so hindsight does not turn a reasonable decision under uncertainty into an unsupported account of individual failure.

Use each question to resolve a gap in that explanation. Ask for the observation or decision context that would distinguish competing accounts, then carry the answer into the causal analysis and prevention plan. A useful review can establish recovery while leaving part of the underlying cause unknown.

Modes

  • Interactive interview (only when requested): review provided history first, maintain an evidence ledger, ask one substantive question at a time and follow its answer. Summarize newly established facts and explain why the next gap matters.
  • Draft/review: assemble what sources establish, challenge unsupported claims and return targeted unresolved questions. Do not require an interview when the evidence already answers them.
  • Recurring review: identify incidents needing review, assemble drafts/question queues, detect contradictory or weak causal accounts and prepare grounded action candidates. Do not invent participant answers, endorse ownership on their behalf or publish a draft as approved.

Consult references/facilitation-guide.md for deeper implementation details when needed.

Facilitation workflow

  1. Ingest ticket, chat/timeline, changes, metrics/traces/logs, actions and stakeholder communications. Normalize timestamps with source/timezone and separate customer impact from response and ticket duration.
  2. Create a fact/claim/unknown ledger and a first causal/response timeline. Read system evidence before asking questions answerable from code or history.
  3. Ask adaptively about the highest-value gap: actual user harm, failure mechanism, decision context, detection gap, mitigation delay, recovery evidence or recurrence protection.
  4. Challenge contradictions neutrally: cite competing evidence, ask what reconciles it, retain both until resolved. Use “what information/control made this outcome possible?” rather than “who failed?”
  5. Analyze trigger, contributing conditions, propagation, barriers, detection, response and recovery. Do not force all incidents into one root cause or a fixed five-whys chain.
  6. Derive specific learning and candidate actions with a causal rationale and observable completion criterion. Distinguish confirmed correction from hypotheses to investigate.
  7. Draft the review in the local format with impact, timeline, analysis, what worked, gaps, unknowns and actions. Mark owner/reviewer confirmation pending where needed.
  8. Review for factual accuracy, psychological safety, useful depth and testable prevention. Hand approved work to postmortem-followup or return the register directly.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

postmortem-facilitation is a skill published in the GitHub repository joao-aires/agent-skills-public (2 stars, last pushed yesterday), licensed MIT. It adds 42 tokens to every session and 775 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-10-15.
