web-a11y-forms

web-a11y-forms is a skill for Claude Code, Codex from klovaaxel/web-a11y-agent-skills. It costs 42 tokens per session (278 once invoked), scanned A, original, MIT.

A guide for building and reviewing accessible forms, including labels, validation, errors, grouping, search, and multi-step flows.

In plain words
What is it for?
Use it to design or audit form-heavy screens, document validation and recovery behavior, and test invalid and corrected input.
Why use it?
It helps prevent forms from confusing people who use keyboards or assistive technology, especially when input is missing or invalid.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00042 $0.00278
Opus 5 $0.00021 $0.00139
Sonnet 5 $0.00008 $0.00056
Haiku 4.5 $0.00004 $0.00028

Measured 11d ago against content hash 254b66707c89, method: parsed. Prices are Anthropic first-party input rates as of 2026-09-11, from the pricing page.

Security

Grade A, and why

web-a11y-forms scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 11d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Web A11y Forms

Form Rules

For label and validation examples, see EXAMPLES.md.

  1. Every field needs a persistent programmatic label.
  2. Placeholder text must not be the only label or instruction.
  3. Use native inputs and controls unless a custom widget is truly required.
  4. Group related controls with fieldset and legend when the group label matters.
  5. Associate help, constraints, and errors with the relevant control.
  6. Preserve user input and focus context after validation failures.

Checklist

Forms checklist

- [ ] Labels are explicit and durable
- [ ] Required/optional state is clear
- [ ] Help text and constraints are discoverable
- [ ] Errors are specific, linked to fields, and recoverable
- [ ] Error summary supports quick navigation when useful
- [ ] Search exposes query context, result count, and no-result guidance
- [ ] Multi-step forms expose current step and progress

Output

Document labeling strategy, validation flow, error recovery, focus behavior, and test steps for invalid and corrected input.

What ships with it

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
