langcare-critical-values

langcare-critical-values is a skill for Claude Code, Codex from langcare/langcare-mcp-fhir. It costs 61 tokens per session (653 once invoked), scanned A, original, MIT.

A clinical alert tool for finding laboratory results that cross urgent “panic value” limits. It uses patient data in FHIR, a standard format for exchanging health records.

In plain words
What is it for?
Reviewing recent lab results, identifying breached critical thresholds, and creating structured notifications with the result, possible cause, immediate action, and monitoring plan.
Why use it?
It helps clinicians spot results needing immediate attention instead of checking every lab value manually. It also gathers related medications and conditions for context.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00061 $0.00653
Opus 5.5 $0.00024 $0.00261
Sonnet 5.5 $0.00012 $0.00131
Haiku 4.5 $0.00006 $0.00065

Security

Grade A, and why

langcare-critical-values scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Critical Value Alert Generator

When to Use This Skill

Use when a clinician needs to identify laboratory results that fall outside critical (panic) value thresholds requiring immediate notification and intervention.

Clinical Workflow

  1. Use fhir_search to pull recent Observation resources (category=laboratory) with interpretation codes HH or LL, or status=preliminary for pending results
  2. Compare each result against CAP/CLIA critical value thresholds (see references/cap-clia-thresholds.md)
  3. For each critical value: identify the analyte, current value, threshold breached, and timestamp
  4. Use fhir_search to pull active MedicationRequest resources to identify contributing medications
  5. Use fhir_search to pull active Condition resources for clinical context
  6. Generate structured alert with: critical value details, probable cause, recommended immediate action, and monitoring plan

FHIR Resources

  • Observation -- Lab results with interpretation codes (HH, LL, H, L, A) and referenceRange
  • MedicationRequest -- Active medications that may contribute to the critical value
  • Condition -- Active conditions providing clinical context

FHIR Query Examples

Pull Labs with Critical Interpretation

fhir_search(resourceType="Observation", queryParams="patient=[patient-id]&category=laboratory&_sort=-date&_count=200")

Pull Pending Results

fhir_search(resourceType="Observation", queryParams="patient=[patient-id]&category=laboratory&status=preliminary,registered")

Clinical Guidelines

  • CAP (College of American Pathologists) critical value reporting requirements
  • CLIA '88 regulations for immediate notification of life-threatening results
  • Joint Commission NPSG.02.03.01: Report critical results in a timely manner

Interpretation Guide

  • Critical values require immediate clinician notification and read-back confirmation
  • For each critical value: state the value, the critical threshold, the clinical risk, and recommended immediate intervention
  • Common critical values and immediate actions: K+ <2.5 or >6.5 (ECG, replacement/Kayexalate), Na <120 or >160 (fluid management, correct slowly), glucose <40 or >500 (D50/insulin), Hgb <7 (transfusion threshold), platelets <20 (bleeding risk, transfusion), INR >5 (hold warfarin, vitamin K), troponin elevated (ACS protocol)

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

langcare-critical-values is a skill published in the GitHub repository langcare/langcare-mcp-fhir (71 stars, last pushed 6mo ago), licensed MIT. It adds 61 tokens to every session and 653 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-30.
