What is langcare-history-physical?

A comprehensive medical history and physical examination document generator for hospital admissions or first specialist consultations. It uses FHIR, a standard format for exchanging health records, to collect patient information.

Creating an H&P with the presenting problem, current illness, medical and surgical history, medications, allergies, social and family history, review of systems, examination, tests, assessment, and plan.

It combines history, examination findings, test results, medications, allergies, and clinical context into one admission-style document. This reduces manual copying between records.
