What is soap-note-generator?

A clinical note generator that creates SOAP notes—Subjective, Objective, Assessment, and Plan—from FHIR encounter data. FHIR is a standard format for exchanging healthcare records, including visits, observations, conditions, medicines, and procedures.

Use it to create primary-care, emergency, or inpatient visit notes from FHIR data, with details such as symptoms, vital signs, lab results, medications, conditions, and procedures. It can optionally save the note as a DocumentReference record.

It organizes information from a healthcare encounter into a familiar clinical note structure. This reduces manual copying of visit details into documentation.

Overview

Generate structured SOAP (Subjective, Objective, Assessment, Plan) notes from FHIR encounter data. Pull chief complaint, history of present illness context, vital signs, laboratory results, active medications, active conditions, and procedures performed during the encounter. Support specialty-specific formatting for primary care, emergency department, and inpatient encounters. Optionally create a DocumentReference resource to persist the generated note.
