source-fidelity

source-fidelity is a skill for Claude Code, Codex from lemos999/codex-active-orchestration. It costs 35 tokens per session (203 once invoked), scanned A, original, MIT.

A source-checking guide for work that depends on reliable evidence. It favors original sources such as official documentation, release notes, standards, research papers, and direct datasets.

In plain words
What is it for?
Use it when researching niche or changing topics, handling high-impact facts, comparing conflicting sources, or preparing work that needs citations. It can also record stable conclusions in an evidence ledger for later implementation.
Why use it?
It helps prevent stale, disputed, unsupported, or incorrectly cited claims from entering an answer or implementation. It distinguishes facts that were checked from conclusions that were inferred or remain unresolved.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00035 $0.00203
Opus 5.5 $0.00014 $0.00081
Sonnet 5.5 $0.00007 $0.00041
Haiku 4.5 $0.00003 $0.00020

Security

Grade A, and why

source-fidelity scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Source Fidelity

Prefer primary sources, official documentation, release notes, standards, repository files, papers, or direct datasets. Use secondary sources for orientation only unless the task is about secondary coverage. For current facts, verify with fresh lookup when the claim could plausibly have changed.

For every important claim, state whether it is verified, inferred, or unresolved. Keep dates concrete. Avoid long quotations; summarize and cite the source locator. If sources disagree, compare scope, date, authority, and evidence instead of averaging them.

When research affects an implementation or artifact, write the stable conclusion into an evidence ledger row and cite that row in downstream work.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
