governed-remediation-recovery

governed-remediation-recovery is a skill for Claude Code, Codex from manishiitg/coding-agent-loop. It costs 36 tokens per session (759 once invoked), scanned A, original, MIT.

A controlled workflow for preparing, approving, carrying out, and checking operational fixes and rollbacks. A rollback returns a system to an earlier working version or state.

In plain words
What is it for?
It helps teams plan remediation, obtain approval, execute recovery changes, roll back when needed, and verify the result.
Why use it?
It adds review and verification to incident mitigation, reducing the risk of making an untracked or ineffective change during recovery.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00036 $0.00759
Opus 5.5 $0.00014 $0.00304
Sonnet 5.5 $0.00007 $0.00152
Haiku 4.5 $0.00004 $0.00076

Security

Grade A, and why

governed-remediation-recovery scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 1mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Governed Remediation and Recovery

Outcome

Create a controlled workflow that turns incident evidence into an exact remediation, validates risk and rollback, obtains required approval, executes through authorized tools, and verifies sustained service recovery.

When to use

Use when a CI/deployment failure or incident has a supported corrective action. Use investigation-only mode when identity, authorization, rollback, or health evidence is insufficient.

Discovery and user direction

Inspect the current workflow, goals, metrics, configuration, capabilities, stores, reports, and triggers before proposing changes. Summarize reusable design and gaps, then ask focused questions for unresolved customer choices such as scope, success, approvals, thresholds, ownership, and budgets. Record the answers as customer direction. Installation alone does not approve workflow changes or execution. Default to one small-team workflow; split only for incompatible access or lifecycle boundaries.

Required inputs

Resolve incident/failure identity, target resource/service/environment, current configuration and revision, allowed action/runbooks, access boundaries, blast radius, SLO/health checks, approval matrix, change window, execution path, rollback, stabilization window, and communication policy.

Plan and AgentWorks tools

Use a message sequence to compare options and prepare an evidence-backed proposal. Persist the exact action, preflight, target state, risk, and rollback as pending review, then end preparation. A separate execution route validates the later durable approval and current target state before protected action, verification, and receipts. Carry review through AgentWorks UI or the configured Slack bot.

Knowledge and persistence

Store proposals, target/config revisions, validation, risk, approvals, execution/rollback receipts, health samples, decisions, and outcomes in durable tables/assets. Keep approved runbooks and constraints in KB context; never convert an improvised command into an approved runbook silently.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
