incident-investigation-coordination

incident-investigation-coordination is a skill for Claude Code, Codex from manishiitg/coding-agent-loop. It costs 36 tokens per session (745 once invoked), scanned A, original, MIT.

An evidence-based workflow for handling operational incidents such as service degradation and outages. It organizes intake, investigation, coordination, and status updates.

In plain words
What is it for?
It helps teams investigate service problems, coordinate response work, and maintain incident status.
Why use it?
It gives responders a shared process for understanding an incident and keeping its progress visible.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00036 $0.00745
Opus 5.5 $0.00014 $0.00298
Sonnet 5.5 $0.00007 $0.00149
Haiku 4.5 $0.00004 $0.00075

Security

Grade A, and why

incident-investigation-coordination scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 1mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Incident Investigation and Coordination

Outcome

Create a governed incident workflow that receives reliability errors, correlates signals, performs basic evidence-backed RCA, establishes impact and severity, coordinates owners, and produces current status without inventing causes.

When to use

Use for alerts, customer-impact reports, deployment regressions, or manually declared incidents. Use lighter failure triage when there is no service impact and no incident policy trigger.

Discovery and user direction

Inspect the current workflow, goals, metrics, configuration, capabilities, stores, reports, and triggers before proposing changes. Summarize reusable design and gaps, then ask focused questions for unresolved customer choices such as scope, success, approvals, thresholds, ownership, and budgets. Record the answers as customer direction. Installation alone does not approve workflow changes or execution. Default to one small-team workflow; split only for incompatible access or lifecycle boundaries.

Required inputs

Resolve services/environments, incident taxonomy and severity policy, SLOs, monitoring/log/trace/deployment/topology sources, on-call and incident roles, trigger mappings, Slack channels, update cadence/audience, evidence access, escalation rules, and closure criteria.

Plan and AgentWorks tools

Use webhook/manual/Slack intake, then a scripted step for validation, deduplication, incident identity, timeline, and evidence queries. Use a message sequence for hypotheses and impact analysis. Branch for escalation, remediation proposal, status update, or insufficient data. Keep one Slack thread linked to the durable incident record.

Knowledge and persistence

Store incidents, signals, affected services, severity decisions, timeline events, hypotheses, evidence, roles, updates, decisions, linked changes, and action receipts in durable tables/assets. Keep approved runbooks, ownership, topology, and definitions in scoped KB context.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
