adr

adr is a skill for Claude Code from me-cedric/agent-compass. It costs 44 tokens per session (523 once invoked), scanned A, original, MIT.

A decision-record writing workflow that turns meeting notes, transcripts, or chat decisions into a MADR record. MADR is a structured format for documenting technical decisions and their alternatives.

In plain words
What is it for?
Use it to record architecture, shared contracts, data ownership, build-versus-buy, and other costly-to-change decisions.
Why use it?
It preserves the chosen option, rejected options, reasons, participants, and follow-up questions so important decisions are not lost.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00044 $0.00523
Opus 5.5 $0.00018 $0.00209
Sonnet 5.5 $0.00009 $0.00105
Haiku 4.5 $0.00004 $0.00052

Measured 10d ago against content hash 86b8a51f27cb, method: parsed. Prices are Anthropic first-party input rates as of 2026-10-07, from the pricing page.

Security

Grade A, and why

adr scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 10d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

adr

Record a decision that is structural and costly to undo: architecture, a shared contract, data ownership, build or buy, a cross-cutting convention. Skip a local choice that the code already shows. For the architecture of a new project, use architecture-advisor.

Steps

  1. Find {{ADR_DIR}}. Create it if it does not exist.
  2. Use the template in that directory when one exists. Its name starts with template or adr-template. Else use the shape below.
  3. Take the next number: the highest NNNN plus one, in four digits. Name the file NNNN-kebab-title.md.
  4. Extract from the source. Do not invent. Take the problem, the forces, each option, its pros and cons, the choice, the reasons and the follow-ups.
  5. Attribute nothing that the source does not support.
  6. Keep every rejected option with the reason that it lost.
  7. Set status: proposed. Set accepted only when the meeting ratified the decision.
  8. Set date to the meeting date. Fill decision-makers, consulted and informed only from the source.
  9. Write each gap as an open question or a follow-up under Consequences.
  10. Link both ways. Link the module README or design note to the ADR. Link the ADR to the spec, the issue and the PR.
  11. Report the path and a one-line summary.

Rules

  • Fill every section, or write N/A.
  • Supersede, never rewrite. A reversed decision gets a new ADR. Change the status of the old ADR to superseded by NNNN.
  • Change an accepted ADR only in its status.
  • Do not commit.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

adr is a skill published in the GitHub repository me-cedric/agent-compass (3 stars, last pushed 4d ago), licensed MIT. It adds 44 tokens to every session and 523 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-10-18.
