coding-rationale

coding-rationale is a skill for Claude Code, Codex from microsoft/cat-agent-skills. It costs 140 tokens per session (1,267 once invoked), scanned A, original, MIT.

A cited explanation for the visit and diagnosis codes already placed on a healthcare claim. It checks which facts in the clinical note support those codes and identifies unsupported elements.

In plain words
What is it for?
Use it to explain why an existing claim code is present or to check whether a note supports the codes already submitted.
Why use it?
It gives a coder or biller a source-based rationale without inventing or changing a code. It helps reveal when the note does not contain enough evidence for the claim.

About the project

microsoft/cat-agent-skills is a static website that catalogs reusable instruction sets and related packages for AI agents. People use it to search, filter, rate, and download skills for Cowork, Copilot Studio, and Scout, along with Copilot plugins and Scout automations. The catalogue entries are the skills, instructions, plugins, and settings displayed by the site.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00140 $0.01267
Opus 5.5 $0.00056 $0.00507
Sonnet 5.5 $0.00028 $0.00253
Haiku 4.5 $0.00014 $0.00127

Security

Grade A, and why

coding-rationale scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2d ago.

The scan reads SKILL.md. This mod also ships 1 executable file (base/scripts/quality_gap_engine.py), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Coding Rationale

Purpose

Give the coder/biller a cited rationale draft for the codes already on the claim - never a proposed code.

When to use

After note-context, whenever claim.visit_level or claim.elements exist - triggered by "why is this coded 99214?", "does the note support this diagnosis?", "build the coding rationale", "what backs up this claim?". Also when the request mixes a rationale with a code change ("bump it to 99215"): build the rationale, decline the change.

When NOT to Use

  • Quality-measure status - use quality-gap-check instead.
  • Drafting patient or referral documents - use document-derive instead.
  • Extracting facts from the note - use note-context instead.
  • Submitting the claim - nothing in this plugin submits anything.

Inputs

note_facts with citations; claim.visit_level and claim.elements (each naming the note_facts key that supports it); config/escalation-policy.json; the user's live message.

Steps

  1. If note_facts are already on the contract with citations, use them; otherwise run note-context first.
  2. Copy the contract to a working file and append the user's message verbatim to messages.
  3. Run the tool: python3 base/scripts/quality_gap_engine.py --question <working.json> --measures config/quality-measures.json --policy config/escalation-policy.json --out <out.json>. Never decide support yourself.
  4. Read back coding_rationale.supported[], unsupported_flags[] and change_requests_refused[].
  5. Report all three lists verbatim - never move an element from unsupported to supported, and never drop one.
  6. Escalate every unsupported element to routees.unsupported_claim_element, and every refused code-change request to routees.code_change_refusal. If the user asked for a code change and change_requests_refused is empty, decline it yourself and escalate it the same way.
  7. Present the evidence pack as a draft with status: "draft" (clinical-doc-rules.md #3). If there is nothing to escalate, say "no unsupported elements - nothing escalated".

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
