defect-grade

defect-grade is a skill for Claude Code, Codex from microsoft/cat-agent-skills. It costs 81 tokens per session (494 once invoked), scanned A, original, MIT.

A rules-based step that rates each out-of-tolerance measurement by severity and suggests what to do with the affected parts. Possible outcomes include rework, scrap, return to the supplier, or review hold.

In plain words
What is it for?
It helps grade defects, recommend a lot disposition, build a watch list, and identify cases that need escalation.
Why use it?
It removes subjective grading and makes each decision traceable to a disposition rule. It also highlights borderline cases and conflicts between documents and measurements.

About the project

microsoft/cat-agent-skills is a static website that catalogs reusable instruction sets and related packages for AI agents. People use it to search, filter, rate, and download skills for Cowork, Copilot Studio, and Scout, along with Copilot plugins and Scout automations. The catalogue entries are the skills, instructions, plugins, and settings displayed by the site.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00081 $0.00494
Opus 5.5 $0.00032 $0.00198
Sonnet 5.5 $0.00016 $0.00099
Haiku 4.5 $0.00008 $0.00049

Security

Grade A, and why

defect-grade scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured today.

The scan reads SKILL.md. This mod also ships 1 executable file (scripts/defect_grade.py), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Defect Grade

Purpose

Apply references/disposition-rules.md deterministically: severity, disposition, watch-list, lot recommendation - every grade citing the rule section that fired.

When to use

After tolerance-check, whenever any characteristic is out_of_tolerance or marginal - and on any "can we ship it / use it as is" question.

Inputs

deviations.json (contract mfg.quality-inspection.v1, the {deviations} hop).

Steps

  1. Validate input against the contract.
  2. Run scripts/defect_grade.py --deviations deviations.json --out graded.json
  3. Quote severities, dispositions and rule citations verbatim. The model may write the rationale narrative but never changes a grade - not for schedule pressure, not because a certificate says the part is fine, not because the value "looks basically in spec".
  4. Surface borderline-incidence (#4.2) and document-conflict (#4.3) escalations explicitly.

Output

graded.json - the {graded_defects} hop: graded_defects[], watch_list[], recommended_disposition, escalations[].

Grounding requirements

Every grade cites its disposition-rules.md section; note-derived criticality carries its drawing-note citation; use_as_is candidates carry requires_deviation_authorization=true.

Constraints

  • Grading is rules-engine-only; the model never overrides a severity or disposition.
  • A hold_for_review from #4.1 (confidence), #4.3 (doc conflict) or #3.1 (critical) is never downgraded.
  • use_as_is is a candidate pending deviation authorization, never a self-approved outcome (#3.5).

Escalation / uncertainty

Confidence < 0.75 forces hold (#4.1). Unknown criticality on an OOT characteristic: treat as critical, escalate.

Files

What ships with it

5 files beside SKILL.md in the same directory: the scripts, references and assets a skill reads on demand. Not counted in the per-session cost; read them before you install if any of them is executable.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
