handoff-draft

handoff-draft is a skill for Claude Code, Codex from microsoft/cat-agent-skills. It costs 161 tokens per session (1,348 once invoked), scanned A, original, MIT.

A clinician-to-clinician handoff drafter for a patient encounter. A handoff is the summary one healthcare professional gives to the next person responsible for the patient.

In plain words
What is it for?
It writes a cited sign-out or handoff for the next clinician or night team, including documented facts, open quality gaps, and coding-related open items.
Why use it?
It keeps open quality issues and unsupported claims visible instead of silently leaving them out. The result remains a draft for the clinician and is not sent or signed automatically.

About the project

microsoft/cat-agent-skills is a static website that catalogs reusable instruction sets and related packages for AI agents. People use it to search, filter, rate, and download skills for Cowork, Copilot Studio, and Scout, along with Copilot plugins and Scout automations. Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00161 $0.01348
Opus 5.5 $0.00064 $0.00539
Sonnet 5.5 $0.00032 $0.00270
Haiku 4.5 $0.00016 $0.00135

Security

Grade A, and why

handoff-draft scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2d ago.

The scan reads SKILL.md. This mod also ships 1 executable file (base/scripts/quality_gap_engine.py), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Handoff Draft

Purpose

Give the next clinician a clear, cited handoff including every open quality gap and unsupported claim element.

When to use

Last skill in the chain - triggered by "write the handoff", "sign-out note for the next clinician", "what should the covering doctor know?", "hand this over to the night team". Also when the request asks to leave an open item out: run anyway, keep every item, and decline the omission.

When NOT to Use

  • A patient-facing after-visit summary or referral - use document-derive instead.
  • Computing measure status - use quality-gap-check instead.
  • Building or explaining claim evidence - use coding-rationale instead.
  • Extracting facts from the note - use note-context instead.
  • Sending, filing or signing the handoff - this plugin never transmits; the clinician does.

Inputs

encounter; note_facts with citations; quality_gap_result; coding_rationale; the user's live message.

Steps

  1. Copy the contract to a working file and append the user's message verbatim to messages.
  2. Always run the tool on the working file, even when quality_gap_result and coding_rationale already exist - results computed before the message was appended cannot record its refusals or escalations: python3 base/scripts/quality_gap_engine.py --question <working.json> --measures config/quality-measures.json --policy config/escalation-policy.json --out <out.json>. Draft from <out.json> only.
  3. If quality_gap_result.halted is "unsigned_note", write no handoff: restate the halt and stop. If coding_rationale.status is "not_applicable", write "No claim on contract" in place of claim items and continue.
  4. Draft the clinical summary from encounter and cited note_facts only.
  5. List every quality_gap_result.gaps entry and every coding_rationale.unsupported_flags entry as one open item each, quoting its fields verbatim and carrying its note_citation (or its rule citation when none exists).
  6. Annotate a refused override on its gap line ("override refused, escalated - still open"); refusals are not counted as separate items.
  7. If omission_requests_refused is non-empty, or the user asked to leave an item out, add one line: "Request to omit declined - every open item stays listed" (escalated to routees.omission_refusal).
  8. Write handoff_note = {"status": "draft", "action_level": "L1", "text": ..., "citations": [...]} for clinician review before it is sent.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
